T-lymphocyte Activation and Chemotaxis in Patients with Atopic Dermatitis

In order to determine whether circulating T cells from patients with atopic dermatitis were also involved in the disease process. Methods:The authors investigated T cell activation markers and the biological behavior. A 48 microchemtaxis chamber was employed to determine T cell chemotaxis activity in 21 patients with atopic dermatitis and 20 normal controls;the expression of T cell activation markers HLA-DR and interleukin-2 receptor (IL-2R) were analysed with fluorescence activated cell sorting technique and serum interleukin-8 (IL-8) levels were measured with ELISA method. Results:T cell chemotactic responses were significantly decreased in patients with atopic dermatitis compared with healthy controls. Increased expression of HLA-DR and IL-2R were demonstrated in circulating T cells from those patients comparied with healthy controls. Serum IL-8 levels werefound significantly increased in patients with atopic dermatitis. Conclusion:Circulating T cells in pa-tients with atopic dermatitis showed abnormal chemotactic response in vitro,which may be associated with activation state in vivo and increased serum IL-8 levels of these patients. The abnormal T cell chemotactic response may be involved in the pathogenesis of atopic dermatitis.

To study the expression of intercellular adhesion molecule-1 in rat heart with ischemia/reperfusion injury and its association with ischemia/reperfusion injury. Methods:Fourty-two male Sprague-Dawley rats were subjected to coronary artery ligation as ischemia/reperfusion heart model and were randomly divided into seven groups of six rats each. The ischemia time was 30 min.The rat hearts were excised at 0,30,60,360,540,720 min after reperfusion. The ischemic and reperfused myocardium was examined immunohistochemically to localize ICAM-1. Malondialdehyde(MDA) and nitric oxide (NO) contents in the myocardium were also determined by thiobarbituric acid and Griss method,respectively. Results: ①At the time of 540 min after reperfusion,expression of ICAM-l in rat hearts was upragulated. ②The MDA values were increased during reperfusion (P<0. 05) and reached the peak 1 hour after reperfusion. ③The NO values get lower and lower during reperfusion. Conclusion :Expession of ICAM-1 in rat hearts subjected to ischemia followed by reperfusion was increased in the late-stage of reperfusion,which might be associated with delayed reperfusion injury.

To examine mutation in the partial coding region of GJB2 gene in 43 cases of Chinese patients with non-syndromic hearing impairment (NSHI). Methods:Genomic DNAs from 43 cases in 2 deafness-mutism schools of Hangzhou and Ningbo,Zhejiang Province,which came from 43 unrelated familes, were extracted and amplified by PCR for a 234 bp coding region of GJB2 gene. The PCR products were applied for single strand conformational polymorphism (SSCP) analysis. Results:At least 10 cases were found to have abnormal shift of bands on SSCP gels with three different patterns. Conclusion :GJB2 gene mutations also exist in Chinese NSHI patients.

To evaluate the types of pulmonary hypertension complicated with large ventricular septal defect (VSD) by Doppler echocardiography. Methods:The ratios of the pre-ejection period to the acceleration period and to the cardiac outputs of right and left ventricules (R PEP: AT :Qp/L PEP : AT: Qs) were measured by Doppler echocardiography in comparison with those of pulmonary to systemic vascular resistance (PVR/SVR) measured by cardiac catheterization in 36 cases of large VSD. Results:The R PEP: AT: Qp/L PEP: AT: Qs measured by Doppler echocardiography were approximate to the PVR/SVR measured by cardiac catheterization in the VSD of the high shunt and the high resistance types,respectively (P>0. 10). Conclusion:The R PEP: AT: Qp/L PEP :AT: Qs measured by Doppler echocardiography can be used to evaluate PVR/SVR and evaluate the changes of blood flow dynamics in two different types of VSD.

To develop a simple, rapid, sensitive and specific dot-immunogold filtration assay (DIGFA) for detection of N. Gonorrhoeae antigen. Methods: The DIGFA kit was prepared by using anti-N. Gonorrhoeae monoclonal antibody as a coated antibody and colloidal gold labelled anti-N.gonorrhoeae IgG as a probe. Results:The kit was tested in 51 clinical samples,there were no cross-re-action with N. Meningitidis, staphylococcus, streptococcus pneumoniae and E. Coli. The minimal concentration of N. Gonorrhoeae antigen detected by DIGFA was 63. 00 ng/ml and the sensitivity 98.04; and the specificity 100;. Conclusion :The DIGFA kit for detecting N. Gonorrhoeae developed inour laberatery is a simple,rapid,sensitive and specific method.

To evaluate the role of serum intercellular adhesion molecule (ICAM-1)changes during pediatric cardiopulmonary bypass (CPB) surgery for congenital heart diseases (CHD)and the clinic significance of monitoring the serum ICAM-1 level. Methods: ICAM-1 levels were measured by double antibodies sandwich ELISA in arterial specimen from 33 CPB cases at various time points,the results were compared with those of 10 non-CPB patients and 30 normal children. Results:Patients with CHD had normal levels of serum ICAM-1 before surgery. The serum ICAM-1 levels in CPB group were significantly higher than those of controls (P〈0. 005). ICAM-1 levels started increasing as early as 30 minutes of CPB began and peaked 24 hours after surgery and stayed up 48 hours after surgery . The serum ICAM-1 levels in patients whose CPB lasted longer than 90 minuets were significantly higher (P<0.05). Conclusion:ICAM-1 showed higher and lasted longer after pediatric CPB surgery. The length of CPB is associated with elevation of ICAM-1. Monitoring of serumICAM-1 ,in certain degree,may help in predicting prognosis and complication after CPB surgery.

Detection of Mycoplasma Pneumoniae in Children with Pneumonia

To evaluate the methods for detection and diagnosis of mycoplasma infections in children with pneumonia. Methods:The polymerase chain reaction (PCR) technique,mycoplasma culture and indirect haemagglutination assay (IHA) were used to detect mycoplasma pneumoniae in clinical samples obtained from 676 children (411 male and 265 female) with pneumonia. Results: Of 676patients with pneumoniae, 115 children were confirmed to be positive of mycoplasma pneumoniae. The positive rate of detection was significantly higher with PCR than with mycoplasma culture (P<0. 01 ). There was no significant difference between the positive rate of detection with PCR and that with IHA (P>0. 05). Conclusion:PCR method and IHA are highly applicable in routine clinical laboratories for the preliminary detection and diagnosis of mycoplasmal infections,particularly in extrapulmonary cases.

Telomerase Activity and hTRT Gene Expression in Gastric Cancer

To investigate the role of telomerase activity and hTRT gene expression in the process of gastric carcinogenesis. Methods:Telomerase activity in gastric mucosa specimens was examined by PCR-based telomeric repeats amplification protocol (TRAP) assay,and hTRT gene expression in gastric mucosa was measured by semi-quantitative RT-PCR assays of hTRT mRNA. Results:Telomerase activity was detected in all of the gastric neoplasm specimens (8/8),and 4 of 10 precancerous state specimens,while no telomerase activity in 9 chronic superficial gastritis. Six of 7 specimens with gastric neoplasms revealed high levels of the expression of hTRT,while the expression was not detected in 5 chronic superficial gastritis. Conclusion: Telomerase activation may be a critical step in gastric carcinogenesis. The detection of telomerase may be helpful in the early diagnosis of gastric cancers. The expression levels of hTRT may correlate with telomerase activity in gastric mucosa.

Surgical Treatment of Thoracolumbar Vertebrae Fracture by Atlas Fixation System

To evaluate the effectiveness and application value of the Atlas Fixation (AF)for thoracolumbar fracture. Methods:Forty patients with fracture of thoracolumbar spine were operated by AF internal fixation. Before operation, the patients were evaluated by the Frankel's calssification;after operation all patients were evaluated again,including X-ray and CT scans. Results:The patients of Frankel's B~D were improved by 1~ 2 scroe after operation. There were no iatrogenic nerve root injury and broken screws. Thirty-six patients had regained the normal spinal sequence. Of 40 cases, 3 had peri-screws abscess and 4 lost the anterior height of spine body. CT scans following-up indicated that the spinal canal volumes were all returned to above eighty percent of normal. Conclusion:Compared with other spinal posterior internal fixation, Atlas Fixation has better results in effectiveness and simplification.

To evaluate clinical effect of percutaneous balloon mitral valvuloplasty(PBMV)in treatment of rheumatic mitral stenosis(MS). Methods:By using Inoue technique PBMV was performed in 336 patients. Results:PBMV was successful in 330 cases of MS with effective rate of 98%,mean left atrial pressure decreased from 3.06± 1.04 kPa to 1.87± 0. 77 kPa(P〈0.01),mitral valve pressure gradient decreased from 2.11±0. 21 kPa to 0. 80±0. 08 kPa(P〈0.01),mitral valve area increased from 0. 96±0. 32 cm2 to 1.81±0. 52 cm2(P〈0. 01) ,and the symptomatic appearance and cardiac function were significantly improved in these patients. There was no mortaliy during the procedure. One patient died 48 hours, after PBMV resulting from low cardiac output (0. 3 ; ). Main complications included perforation of cardiac wall with tamponade in 7 cases(2;), middle mitral regurgitation in 6 (1.70; ) ,iatrogenic atrial septal defect in 7 (2 ; ) and cerebral embolism in 2 (0. 5 ; ). PBMV was successfully performed again in 10 patients. Conclusion:In treatment of rheumatic MS PBMV has good clinical effects,and even for critical,complex MS or restenosis after PBMV it may also be one of valuble choice.

Microsurgical Treatment of Arteriovenous Malformations of the Corpus Callosum

To review the therapeutic experience of arteriovenous malformations(AVMs)of the corpus callosum. Methods:Eight cases of callosal AVMs admitted to our department from Jan.1990 to Dec. 1998 were reviewed retrospectively. Results:One patient who refused surgical operation died of rebleeding six monthes late. Other 7 cases were operated on microsurgically including total resection in 5 cases and subtotal in 2. One case also underwent endovascular embolization during operation. There was no operative mortality. Five cases were followed up(mean 26 monthes), all had good recovery. Conclusion :When arteriovenous malformations of the corpus callosum are diagnosed,microsurgical excision should be done. Combination of endovascular embolization with surgery is acceptablein management of difficult cases.

To elucidate the value of percutaneous endoscopy gastrostomy(PEG) under endoscopy in patients with severe head injury. Methods:Seventeen patients with severe head injury whoneed to be supported with enteral nutrition had undergone PEG. Results: Nutrition status hadmarkedly improved for the survival patients. Three cases died due to the initial insult, 2 had rednessand secretions at the PEG wound and no other complications of PEG were observed. Conclusion:Forpatients with severe head injury long duration of enteral nutrition support via PEG are preferable.
