微波组织凝固器应用于肝脏手术的实验研究

作者用微波组织凝固器对离体猪肝行微波辐射,并对其剖面作红外摄象,发现微波辐射后组织内的热像图有从上宽下窄的楔形演变为椭圆形的过程;微波辐射时间与组织中心温度升高呈直线回归关系,含水量高的组织(Y=14.35+0.62x)其微波热效应明显强于含水量低的组织(Y=12.42+0.56x,P<0.05);微波组织内透热半径呈指数曲线下降(Y=116.7e-0.96x)。在活体家兔中用微波组织凝固器行肝叶部分切除术,并以电刀切肝作对照,结果显示:微波组出血量(6.12±1.71g)明显少于电刀组(13.05±2.16g,P<0.005),且前者并发症少;微波辐射野血管痉挛、内皮细胞破坏、血液凝聚、血管栓塞;血清酶的升高在术后2~4天恢复正常。笔者认为:微波组织凝固器应用于肝脏手术是较安全、有效的。

Abstract:

A microwave tissue coagulator(MTC) was inserted into a piece of swine liver with different water contents and the thermograph of tisssue was recorded by the Probeye thermal video system.A linear regressive relationship was revealed between the rise of central temperature in the tissue and microwave radiation duration. The effect in the tissue with high water content was much stronger than that with low water content (P<0.005). The penetrating radius of microwave radiation attenuated exponentially (Y=116.7e-0.96x). As the radiation continued, the wedge-shape of tissue thermograph changed to ellipse-shape. In a controlled study on rabbit hepatic resection with MTC or cautery,the blood loss of the MTC group was lower than that of the cautery(P<0.05). No delayed liver stump bleeding and bile leakage occured in the MTC group.The histologic findings indicated a similar healing process at the resected liver stump in the two groups. Postoperative increase in activities of SGPT and AKP was transient in both groups. These findings led us to conclude that MTC could be applied safely,effectively and easily in the field of hepatic surgery.
